New technique for treatment of patients with coronary artery disease was introduced at our Center

At the beginning of April, a new method based on assessment of functional significance of coronary artery stenosis by measuring instantaneous wave-free ratio (iFR, instantaneous flow reserve) was introduced in the Department of X-Ray Endovascular Diagnostics & Treatment No. 1 of our Center. Several surgeries were carried out jointly with a team of specialists from National Medical Research Centre of Cardiology named after Academician E.I.Chazov under the guidance of Vsevolod Mironov, MD, Ph.D.

Currently, treatment of coronary artery disease is intended for selection of technique that will be the most effective and safe for the patient. Each treatment option has its own indications and contraindications based on international guidelines. An extraordinary group is patients with lesions of the trunk of the left coronary artery and multiarterial lesions of coronary arteries.

Each patient needs an individual approach from specialists and a clear justification for treatment technique. Especially for this purpose, cardiologists use various tests and methods to assess the condition of a damaged coronary artery. This helps to observe the picture completely and more accurately evaluate how damaged an artery is, and thus to avoid unnecessary surgeries and risks associated with them.


Stanislav Sapozhnikov, MD, Junior Scientific Researcher, specialist in x-ray-endovascular methods of diagnosis and treatment shared that in modern endovascular cardiology, the key positions were occupied by techniques aimed at assessing coronary physiology. The highest category in the national guidelines for treatment of patients with chronic forms of coronary artery disease was technologies for measuring the fractional flow reserve and iFR. He said that for many years at our center we had used the first technique. However, iFR had important advantages. For example, patients came through the procedure easier. At workshop given by colleagues from Moscow, we applied the new technique for the first time. This made it possible to accurately identify lesions causing myocardial ischemia and to successfully perform targeted stenting in patients with multiarterial coronary artery disease.


It should be mentioned that our center has a special device and sensing system for such procedures.
