Tomsk NRMC scientists have developed new approaches to increase the effectiveness of treatment among patients with acute myocardial infarction

At Academic Council meeting of Tomsk NRMC, Ivan Bessonov, MD, PhD, Head of Laboratory of X-Ray Endovascular Diagnostics & Treatment of our center presented a report: "Ways of result optimization in treatment of patients with acute myocardial infarction with ST segment elevation."

Acute myocardial infarction in industrialized countries, including Russia, is one of the leading causes of death in population. Urbanized lifestyle influenced on it: low physical activity, high-calorie nutrition, smoking and psycho-emotional stress lead to the development of atherosclerosis and subsequent cardiovascular accidents.

The cause of myocardial infarction is most of the time blockage by blood clot in artery that supplies blood to heart muscle. The modern approach to the treatment of patients with acute myocardial infarction is aimed at the rapid restoration of arterial patency. Today, surgeons can act on inside the coronary artery, through a small puncture in skin. As a result, the patency of the artery is restored, and a miniature prosthesis-coronary stent, is implanted in place where the blockage occurred. The method is called "percutaneous coronary intervention." Thanks to this approach, many countries have been able to significantly reduce the mortality rates of patients who have had acute myocardial infarction.

Researchers our Center set a goal: to identify predictors of adverse outcomes after acute myocardial infarction and to develop approaches to improve the effectiveness of treatment of such patients.

Ivan Bessonov mentioned that in 2006 our Research Center became one of the first medical organizations in Russia that performed percutaneous coronary intervention (PCI) for patients with this diagnosis on a 24-hour basis. Currently, the number of patients who underwent PCI for acute myocardial infarction with ST elevation at our Center exceeds 2000 people. Presence of such a large database allowed our scientists to conduct comprehensive study of the problem.

Bessonov shared, researchers studied and evaluated such factors of hospital mortality as age, location of myocardial infarction, total time of myocardial ischemia, unsuccessful PCI, patient hyperglycemia at admission, SYNTAX scores and severe heart failure.

As a result, scientists outlined prospects that will significantly increase treatment effectiveness of patients with acute myocardial infarction.

Methods with good prospects for improvement effectiveness of treatment of patients with acute STEMI are elaboration of techniques for increasing angiographic success concerning PCI rate, reducing the area of myocardial infarction with anterior localization, as well as appliance of mechanical circulatory support devices in severe acute heart failure. Ivan Bessonov commented the importance of microvascular obstruction syndrome prevention in order to achieve angiographic success in percutaneous coronary intervention. To do this, scientists have developed a special scale that can be used to identify patients with a high risk of developing microvascular obstruction syndrome with predictive accuracy.

According to the results of the study, 22 articles, 1 manual were published, 2 patents were registered and 1 computer program was developed. Members of Academic Council of Tomsk NRMC drew attention to importance of the issue and relevance of the presented report, appliance of up to date technologies in our center and detailed analysis of data.
