In the end of the last year interventional cardiologists of Tyumen Cardiology Research Center performed first operations using modern technology of rotational atherectomy (rotablation). Yesterday this kind of operation saved life of an Ekaterinburg citizen, when other ways of CAD treatment did not work.
Mikhail, the patient, is 81 and during 50 years he has been working as an electrical engineer. His health has always been proper but recently he started to feel chest pain, but only a week after he applied to a cardiologist. Cardiology center in Ekaterinburg performed coronary angiography which showed that one vessel is closed with the atherosclerotic plaque for 85% and another one – 95%. One plaque was enhanced with the balloon but the other did not get broader even after the sixth try. Ekaterinburg cardiologists informed that the only way out is coronary artery bypass grafting. Mikhail said, ‘No one told me about another treatment, but together with my daughter we found more mild treatment in Moscow and Tyumen. We decided to apply to Tyumen as it is closer by distance.’
After three days of hospitalization he had an operation performed; now he is fine and next week will be discharged. He said that during the operation he stayed calm, there was no pain or discomfort, he could watch the monitor and answer the questions. ‘I could thank the surgeon right after the operation! I wish our doctors gave all the information about such a low-trauma interventions.’
Rotablation is prescribed for patients with coronary artery disease, vessels of which are damaged by calcific atherosclerotic plaques. Calcium decreases flexibility of the artery wall, therefore standard interventions like angioplasty and stenting are almost impossible to perform. To avoid strokes a patient needs a complicated open coronary artery bypass grafting. Whereas rotational atherectomy is performed using a special bur with diamond coating. Making from 180 up to 220 rotations per minute, bur bores a thick plaque out. When the lumen is open, surgeons can install a stent in the artery and avoid re-narrowing of the vessels. As all endovascular interventions, operation is ongoing under the X-ray control with special guides – catheters. Duration is about two hours. The procedure refers to hi-tech treatment and is financed by the regional and federal budgets.
